covered by any kind of health plan? Have you ever purchased prescriptions
outside of the United States because they were available for less or not
available in the U.S.? If so, I'd like to tell you about an upcoming conference
call we're setting up to discuss healthcare issues facing Americans today.
This is for research purposes only and will not result in any solicitation
of any kind. Participation is confidential and participants on the call will
be first-name basis only. Qualified participants will receive an honorarium
for $75 to thank them for their time. This can be send directly to the
participant in their name as a personal check or we can make it out to a
charity you designate in your name.
Qualified participants will receive $75 to thank them for participating in a
90-minute call. Participation is completely confidential and on a first-name
basis only. The discussion is strictly opinion-oriented and concerning
healthcare issues.

2. Your highest level of education completed
3. Do you have a chronic condition or do you assist in healthcare maintainance for a family member? What condition(s)?
4. Have you ever purchased prescription medications for yourself or a family member over the Internet (if so was this covered by any kind of health plan?)
5. Have you ever purchased prescription medications for yourself or a family member in another country (if so was it because you happened to be living or travelling there at the time, or did you go specifically to purchase a medication that was either not available or more expensive in the U.S.)
6. Your daytime and evening contact numbers (both if possible).
